Our stairs have been painted for many years and through several owners. I've gotten used to having to touch them up every year, but now the different layers of paint are becoming too uneven.

At first, I thought about renting a concrete hand grinder and sanding off basically all the paint I could reach, but now I'm starting to have doubts. There's a risk that some concrete might come off if/when I wobble.

Is there any type of sandpaper that works or a diamond grinding disc for a regular 150 mm sander (Festo)? Regular paper usually doesn't sand well and just clogs up!

Considering the dust that is generated, you need a grinder with a shroud/dust cover and extraction for a vacuum cleaner. Rent a concrete grinder. It is not that effective. Be prepared for the disc wear to cost a fair amount.
 
Sanding paint in thick layers poses a significant risk of quickly becoming smeared. Have you considered scraping or sandblasting?
