I think thresholds are quite ugly, and must have them where two floors meet at different levels. Looking at thresholds in the market, they are full-size, i.e., cover the entire depth of the door frame. I have a hand router and am considering making my own, narrower threshold out of oak. Is there anything I should consider? (I might make one for the bathroom as well)

The door frame's molding is cut about 20mm from the floor, and theoretically, I could "squeeze in" the threshold under the molding so that the threshold puts pressure down towards the floor. However, I will probably need to screw the threshold in place anyway.

I have also looked at the custom steel profiles available in the market, but I don't think I found one that suited my measurements. I will make another attempt and see if I find something.
 
An image would make it much easier to give you advice.

A narrow threshold that floats on the floor and is screwed into the subfloor should work.
 
There are also wooden strips that accommodate height differences, but there's nothing stopping you from making one yourself.
