Tips, Change the question "do you remember which rotary hammer drill you used". I assume you want to know the brand or model. But as the question is formulated, an aspie Engineer interprets it as a yes or no question. So, "do you remember the brand, yes".

I also think you’re missing some questions about ergonomics. As you have set it up now, there are more questions about the test person than about ergonomics. I think it is more reasonable to understand what could be an ergonomic problem before trying to find out if it is men or women, age, profession, etc.

The questionnaire takes about 30 seconds, by the way :) 5 minutes is a bit much!

I thought we would try to reach as many people as possible and that's why some different threads were created, but it seems to have ended up in the same place, so that was unfortunate! Thank you for pointing that out!
 
What is actually the purpose of the survey?
 
As constructive criticism, I want to point out that, just as Snickarkirre asks above, it's good to share the purpose of the survey so that respondents know what they are answering. Likewise, it should be backed with names and contact details (especially if it's at college/university level).

And even if this is quite unscientifically conducted (not a thoroughly worked-out form thrown on a forum online), it's reasonable to demand that compound words are written together.

Or maybe I'm just on my way to really becoming an old man..

That said, I believe you would have gotten significantly better results and material to work with if you managed to secure a smaller number of subjects for more in-depth interviews. A couple of craftsmen and a couple of private individuals. Then you could have received more detailed information about how choices are made and why certain methods are used. You could also illustrate with pictures. But then again, I don't know what your survey is supposed to lead to, so maybe that's not the right direction to take?
