I want to replace this trellis with a railing in turn-of-the-century style but I don't know what to do with the wide, low "pillar". Does anyone have tips on how to solve this?

Your suggestion fits well in a house from the turn of the century and resembles what I had. The bottom edge seems to be original with you. There is no reason to have slats up to the ceiling.
