I have installed a couple of doors in the basement where it was necessary to saw upwards to fit a standard size. But to avoid larger modifications, I decided to only saw up right where the door stands like this:

If you look closely here, you can see that the lintel protrudes about 1 cm, creating a clearer framing of the door.

The later door that was installed ended up 1 cm higher (due to the floor), and it turned out like this:

You can see that the lintel is completely hidden, almost to the point where the concrete hangs down a bit in front.

How should one think about this? Can it be considered an okay installation, or should something else be corrected? Chip out the vault more?

You can lower the entire frame if you let the threshold be level with the tile floor.
I would probably have done that and made my own threshold that goes up a bit just before the klinik & goes over the klinik a bit.

Would have also done as written above.
Cut out the tiles and lower the door to not have such a high threshold.
Or if you don't need a threshold, remove it completely and cut the frame a bit to lower it and make it completely smooth.
You can continue with tiles in the next room at the same height.

Depends on what kind of room it is.

The door leads into a workshop in the basement which usually has a lower set temperature. For that reason, I have installed a "standard" threshold so that you can attach a sealing strip to the door.

But it sounds like you think it's okay to embed the threshold between floor A and B? There's no direct reason for it to lie on top of the floor?

At some point in the future, we will also lay tiles in the workshop, but it's possible that the floor height won't be the same there (15 mm).

If you need to move a door that's already installed with screws in hollow concrete blocks, how do you do it? The stone will likely crack if you drill holes 1-2 cm below the existing holes.

Haha yes, the door was installed well, but in hindsight, I think it's a bit high. The rest of the family feels it's fine, but I'm not entirely sure. So my thought here was to see if there are any guidelines to follow when installing a door, or if you just place it as you like, resting on the existing floor or between two floors.

If one were to move it down, how do you actually do such a thing? All I can think of is getting a syringe and injecting some type of hard filler into the existing holes, wait for it to dry after a couple of days, and then drill new holes?
