But how do I get 45mm insulation from the ceiling that then creates an air gap up to the ridge?

Either you nail moldings to the rafters and then masonite boards on these, or you go for the slightly smoother alternative wind deflectors in corrugated cardboard http://www.t-emballage.se/sortiment/start#artikel=265002
