Is there anything that prevents the box from rotating? Well, the hose/pipe is also fixed, so I assume everything holds together.

If you don't nail it down properly with the nail or happen to set it a bit crooked, concrete can come between the box and the mold, but it's easy to chip away since it forms such a thin layer.

Then it might be that if you nail it onto the mold you lift into place (you have one side against which you reinforce and the other side you set in place last), there's a risk you might hit a reinforcing bar that nudges the box or crushes the box, but then you really have to apply force and force the mold into place ;) for that to happen.

When you remove the casting form, do you just pull it away and the box cover follows and stays in the form?

Pull away the mold. The 'doslock' usually stays, but some of the nails may remain in the mold.
