How should I solve the installation of a handrail?

Hi, going to install a handrail.
See pictures. There is a ledge, a platform 10 cm from the stairs to the wall.

Normally, the handrail starts about 10 cm in on the wall at the beginning of the stairs.

What to do here? Some kind of spacer? Starting in the middle of the stairs feels strange, doesn't it?

And would you put a handrail on the short wall as well, as usual?

There are extensions for certain brackets, and some brackets come in different lengths from the wall. However, I think I've only seen a 5-6cm difference. But a thorough Google search might find more. Otherwise, the alternatives are to use a spacer or ask a blacksmith to modify a bracket.
