If we are going to install a pair of doors that are 240 cm high and 116 cm wide excluding frames, how large does the opening in the concrete need to be?

How much should be added for the frames?

Do you need to add for wooden studs as well, or can you place the frames directly against the concrete?

It is difficult for us to answer, partly because you seem to have unusual measurements for the doors, and over the years there have been different brands, each with their own profiles..
Do you have frames or are you going to manufacture frames for your door leaves?
Normally there is a gap of 10-15mm around the sides and the lintel..
The frame is fastened in the concrete with frame screws or Adjufix frame sleeve if you want it extra secure..

A 1 cm gap around the frame is probably just right, and here you need to know your frame thickness to be able to calculate the hole. You can attach the frame directly to the concrete, but I would probably make the hole big enough so that I could also fit in a stud to screw the frame screw into. I believe it will be easier to adjust then, at least if you use, for example, a frame screw from Essve with a sleeve.
