I'm planning to renovate the guest toilet and I want to improve the soundproofing as well.
The walls are made of some kind of masonite-like board with paper studs (similar to an interior door) and no insulation.

Is it worth replacing the masonite with plasterboard and adding some insulation, or would it be enough to just screw plasterboard directly onto the masonite?

If it is soundproofing you are after, you should probably insulate inside the wall. If it's more about general refreshment, then plaster or other surface renovation might be enough.
 
Depends a bit on how much space you want to sacrifice. Soundproofing gets better the heavier the wall is. Double plasterboards make it significantly quieter and if you have space, it is easier than tearing down the existing wall construction.
