Have a narrow staircase with walls on both sides. About 75cm wide. (Old attic staircase).

I don't want handrails.. Is it an absolute requirement, even if there are walls on both sides, and it's so narrow in between???
It's a renovation under a building permit, so I will have a site visit from the Building Inspector before the final approval.

Thick rope I have seen in some museums and similar places that are in old fortresses and the like.

Maybe recess the handrail into the wall? Rope/stair rail line as a handrail?
Recessing is not possible, I have a sliding door that goes in the last upper 80-90 cm of the stairs.
