Previous owners have made a doorway in the basement in what I assume is a load-bearing wall. The wall is made of concrete blocks and the opening has been there for at least a few years. The floor upstairs (a single-story house built in 1946) slopes a bit above this doorway.

Is this a sufficient load-bearing structure, or should I be worried? Is it common to use flat iron like this?

Late reply from my side, sorry, but how should I proceed to redo this? It has settled so I probably need to raise it a few millimeters and then brace it correctly.

I haven't been able to see if there is a beam supporting itself over the doorway, but I hope not. But the fact that it sags a bit might indicate that there is a beam not getting any support?
 
I would guess that there is a floor beam right above the door since there is a row of nails in the ceiling there. It looks like the wall above the door has started to crack a little.

Yes, maybe. However, the row of nails is beside the opening on the other side of the wall. And there are quite a few strong rows of nails in the ceiling, so this should be one of many load-bearing floor joists. Having one over the opening in the heart wall might not be too problematic, except for possible settling. It seems to have stopped though, as there hasn't been any change in 6 months anyway.
