I was going to put up a shelf the other day and when I was tightening the screw with the drill, the entire screw head came off. The rest of the screw is still completely inside the plug... Does anyone have an idea on how I can get it out? Tried to twist the screw out with pliers with a narrower shaft but it won't budge.

What kind of wall is it?
Depending on the type of wall, it might work to punch it in with a drift pin and then insert a larger plug and larger screw.

It is concrete, or maybe lightweight concrete, an exterior wall in the apartment.

I might have drilled it out, or maybe used a torch to melt the plastic. There are probably easier ways :)
 
I had moved the shelf slightly to the side and made new holes.
If the old holes are visible, take a screw and tap the old one slightly into the wall, then fill and paint.

Yes, I think that's almost the simplest option... it's actually a nightstand/shelf, and the problem is that I put up the first shelf on the other side of the bed before this one, so if I move this one a little it won't be symmetrical. But I'll have to live with it :)

Tried that, but unfortunately couldn't get it loose.
 
What kind of plug is it then? It looks like the screw is too large for the plug and maybe it's too tight and that's why it broke.........

If it's tightened so much that it broke, a wrench probably won't get a good enough grip to remove it. Maybe if you spray some 5-56 to reduce friction against the plug.

Is it possible to make a new mounting hole in the shelf so you can place it where you want?

Take a fine drill and drill away as much as possible of the plug and then pull/screw out the screw with a suitable pliers.
