When was the house built? Do you have any blueprints? I think the slab looks too dry to be uninsulated.

Drainage is not a critical issue in this context. If the ground is of a draining type, drainage pipes are not necessary. Moisture in foundation slabs is almost always a result of the absence of insulation underneath. Then, water vapor from warm indoor air condenses in the slab. I think there is a lot to suggest that the slab is insulated. In that case, you can place insulation directly on the concrete. The best would be to have a drawing to confirm this.

Okay, thanks for the response, I might chase down a drawing. When I tore it up, I didn’t see anything noteworthy, but the threshold to the balcony door had absorbed moisture, which is why I opened the floor.
 
Was there something wrong with the loose fill insulation that was there? If it's dry and fine, you can just continue with the same concept, perhaps just sealing a bit more outside the walls and supplementing with more equivalent material.
