I was thinking of tiling a bit in the boiler room, the walls are made of plastered brick blocks that have been painted with some unknown white paint, both the plaster and paint seem to adhere well.

The question is whether anyone has experience tiling on painted surfaces? Alternatively, plaster over painted surfaces and then tile?
I've been suggested to rent a diamond grinder and sand all the walls, but that suggestion isn't overly appealing...

Yes, sanding is probably the only thing I can think of in this case to ensure that the result is good. Whether it's silicate, kalk, kc, or plastic paint, it will be a botched and half-hearted job if you just slap on a new surface layer.

My opinion though.
 
As long as it is securely fastened, there shouldn't be any problem. Wash with paint cleaner then apply a strong, good primer. It's good to use a wire brush to see if the paint holds. It's the paint that can cause issues...
