The plate should be installed before the trim and go underneath as it is done on all window sills:

Are those two different windows you photographed? I can't make sense of it.
 
The first image is a door threshold, the second image a window.

The first picture shows how it looks under all doors, patio doors, and windows that go all the way down to the slab. Incorrectly installed in my opinion.
The second picture shows how it looks at all the windows. Correctly done, I think.
I just want agreement before I start the process against the carpenter.

The window is right, but the door is wrong.
To be able to make a correct bend on a flashing, it must have a back edge. The back edge also makes the flashing more stable.
