What is it really like?

from the inside...
...
- 195 isolated frame, vertical
- 70 insulation but horizontal battens that break the thermal bridge
- wind barrier
- 28x70 nailing batten for air gap
- vertical clapboard.

The horizontal nailing batten must then be mounted flush against the horizontal batten all the way. Is that how it's done? Or should you put some spacer between the nailing batten and the batten, like a piece of paper?

Are there any disadvantages to horizontal nailing battens that are nailed directly to a horizontal stud?

Shouldn't be any problem doing that. When I added insulation to the house, I installed horizontal studs, put on a wind barrier, and then horizontal battens on top of that.
